Foreign students cannot apply for a leave of absence through the online portal. 

Therefore, you must complete the attached form and submit it in person as a paper document.

Please follow the steps below:

1. Fill out the form.

2. Visit your department office to obtain the signatures/seals from your Professor and Dean.

3. Visit the Division of International Service to obtain their seal.

4. Finally, submit the completed form to the Division of Academic Affairs.
